Medical Records Upload

If you have previous veterinary records, lab results, or vaccination history, please upload them here before your pet’s appointment. Having access to your pet’s medical history allows us to understand their needs better and provide safe, informed care during our visit.

Accepted file formats include PDF, JPEG, and PNG. All records are reviewed by Dr. Mahlberg directly and kept confidential.

Once your documents are uploaded, our team will confirm receipt and contact you if any additional information is required.
